The Seminary offers health insurance through Aetna with employee's having the option to choose one of four plans. This page includes all of the information you will need to make an informed decision about which Aetna plan is best for you. Plan Summary Designs

A plan summary design is a document that insurance providers are legally required to prepare and it includes different types of services a person may access and the cost associated with these services. Examples of services would be visiting a primary care physician or a specialty care physician, going to urgent care or the emergency room, or filling prescriptions.

Title Description File type

This is the plan summary design for the Aetna PPO 1. This plan has a $1,500 individual and $3,000 family deductible. This is national coverage.

This is the plan summary design for the Aetna PPO 2. This plan has a $2,500 individual deductible and a $5,000 family deductible. This is national coverage.

This is the Aetna plan summary design for the High Deductible Health Care Plan (HDHP). The individual deductible is $2,800 and the family deductible is $5,600. This plan is specific to Northern Illinois.

Flyers on Aetna's Value-Added Services

These flyers provide information on the additional services provided by Aetna to promote your health, wellness, and emotional wellbeing.

Title Description File type

This is an 8-week behavioral health program. Once each week you meet with a therapist to address emotional challenges like depression, stress, and anxiety that can come with a medical diagnosis. A second time each week you meet with a behavioral coach to identify health goals and develop an action plan. There is no additional cost for this benefit.

Attain by Aetna app combines your health history with your Apple Watch activity to offer personalized goals, achievable actions, and big rewards — like an Apple Watch or gift cards.

Employees can seek service at Minute Clinics found in CVS and Target stores at no cost. Please note the fine print in the flyer. Those on an HDHP, who have not yet met their deductible would have to pay out of pocket. Those on PPO and HMO plans would have no cost.

Your Personal Health Record (PHR) lives on your Aetna member website and is powered by their patented CareEngine® technology. It scans the information in your personal health record and compares it to thousands of the latest medical guidelines. If PHR spots potential medical problems, drug interactions or gaps in care (like missed tests or procedures), it’ll send you and your doctor an alert.

This is a demonstration of how to elect, change, or waive benefits in ADP. This video is meant for employees who do plan to elect benefits.

If you plan to waive most benefits, this is a short video demonstration of the ADP Open Enrollment feature showing you how to waive benefits and submit your enrollment.

Shortly after November 1, 2020, you will be able to visit https://www.aetna.com to register and sign in to your account. Around that same time, you can text "AETNA" without the quotes to 90156 to receive the link to download the Aetna Health mobile app. Messaging rates may apply.

Aetna DocFind

You can use this link to access Aetna's DocFind as a guest. https://www.aetna.com/dsepublic/#/contentPage?page=providerSearchLanding&site_id=dse 

  • In the continue as guest section, enter your zip code and then use the slider to select the mile radius that would encompass how far your doctor is from the zip code entered.
  • On the next screen, you need to select your plan type. If you would elect the Aetna PPO or HDHP then under Aetna Open Access Plans, select “Managed Choice POS (Open Access).” If you would elect the HMO plan with Aetna, under Savings Plus Plans select “Savings Plus of Northern Illinois (HMO).”
  • Then you will get to a screen where you can search by doctor name or you will see you can also do a general search by clicking on medical doctors and specialists, hospitals, behavioral health, etc.

Prescription PDFs

To find out what tier your prescription drugs are in, please open this PDF. It is more than 200 pages, so press CTRL + F and a box will appear where you can type in the name of your prescription and hit enter. This will then find your prescription name in the document. From there, you will look at the abbreviation in the "Drug Tier" column. Here is the key:

CE = Copay Exception: Available to some members at no cost with a prescription from your provider when obtained at an in-network pharmacy. Certain limitations may apply.
NF = Non-formulary, not covered unless exception request granted
NP = Non-Preferred Brand and Generic
NPS = Non-Preferred Specialty
PB = Preferred Brand
PG = Preferred Generic
PSP = Preferred Specialty

You would then look at the PPO, HDHP, or HMO plan designs to see what you would pay for your prescription(s). You will find the drug tier information on either page 2 or 3 of the plan summary designs.
